Please read these terms and conditions carefully before availing Dr. Sandeep Sattur and Dr. Indu Sattur’s online consultation services.
You hereby agree and consent to the following:
I confirm that I am 18 (eighteen) years or older and am eligible and competent to provide my consent herein. I also confirm that I am competent and eligible to avail consultation with a doctor as per applicable laws. I authorize Dr. Sandeep Sattur and Dr. Indu Sattur (collectively, referred to as “Doctors”) to assess my medical history and to provide healthcare services on ‘as is’ and ‘as available’ basis, including administration of drugs/medications as deemed necessary. I am aware that healthcare services will be provided through telephonic or Internet consultation (audio/video/email) with the Doctors (“Consultation”) and that there will be no examination in person. I agree that the diagnosis based on telephonic consultation will be at a pre-primary level and that I will visit the Doctors for an in-person consultation as advised. During the course of the treatment I will disclose sensitive personal information (“SPI”) which may include without limitation (i) physical, physiological and mental health condition, symptoms and history; (ii) medical test results in connection with the aforesaid; (iii) medical records and history; and (iv) other personal details such as name, date of birth, postal address, e-mail, telephone number, family’s medical condition and biometric information, to the Doctors, which the Doctors may store and use solely for the purposes of my treatment. The Doctors will not publish and disclose the SPI to any third person or body corporate without my express written consent, except as required by law. I may review the medical history and other related records provided by me to the Doctors and request the Doctors in writing to correct or amend any inaccurate or deficient information. The Doctors will not be responsible for the authenticity of the SPI provided by me to the Doctors. I hereby consent for audio- video recording of tele-medicine consultation and that Doctors may retain complete recording of audio/ video/ email/chat conversation as required under applicable law. I agree that the Doctors’ liability will only be limited to the professional services rendered by them and that the Doctors do not make any guarantee, representations or endorsements or implied or express warranties with respect to any part of the services provided by them. I agree to pay and transfer the consultation fees, as intimated to me, prior to the appointment being made and the Doctors will not be responsible for any kind of failure/delay of such payment transaction. Further, I agree that the Doctors shall not be liable, in any circumstance whatsoever, for any harm or complication arising to me or my electronic devices due to any mobile/internet applications used by me for the purposes of and during the course of the Consultation.
In the event, this consent is being provided on behalf of a minor or in the capacity as a guardian, I agree that such confirmations and acknowledgements as provided in this consent are true and I am legally able to provide the aforementioned confirmations and acknowledgments on behalf of such minor.
The Doctors do not provide emergency medical services and are available for consultations in relation to hair, scalp and skin problems. If you are having a medical emergency, please call your local emergency contact number.
By using the services of the Dr. Sandeep Sattur and Dr. Indu Sattur you agree that you have understood and agree to all of these terms and conditions and the ‘Telemedicine – Terms of Use’ available at www.hairrevive.com, and consent to receiving phone-based or internet-based consultation and information from them. The Doctors reserve the right to make changes to these terms and conditions.
The terms and conditions and the services will be governed as per the applicable laws of India and the courts of Mumbai (India) only will have exclusive jurisdiction over all or any matters arising out of or relating to these terms and conditions or the services.
